Child Passport (DS-11)
If your child is 15 years of age or younger follow these steps:
You can use the Statement of Consent Regarding a Minor (DS-3053) form if:
One of the legal guardians or parents cannot be present at the acceptance agent. Parent not present must fill out and have notarized and also provide photo copy of drivers license or state ID.
When Application is complete Create Form then Print
Only if your child was issued a U.S. Passport in the past and can't find it, in addition to DS-11 you must complete Form DS64

2 Copies of Travel Itinerary
Check or Money Order in The Amount of $140.00
Authorizing Prompt Passports to Expedite Your Passport
Passport Photos Can be Taken at CVS, Walgreens, Rite Aid or Post Office
ID Must have Issued Date of 6 Months or Longer
Birth Certificate or Naturalization Documents Will be Returned to You With New Passport
2 Photo Copies of your travel itinerary showing your name and date of travel.
Business Travelers provide letter from employer outlining immediate travel.

In order to verify your identity and maintain security when applying for a U.S. Passport, all passport applicants are required to show up in person at their local passport acceptance agent. The agent will put all your documents in sealed envelope and return to you. These agents are located at Post Offices and County Clerk's Offices across the country. There will be a $25.00 fee
